How to Organize IBD Medical Records and Documents
Medical records and paperwork can arrive from different offices, portals, and insurance contacts. A clear organization system gives you one place to find the documents, dates, and administrative notes you already have without asking you to interpret them or make care decisions.

Begin with a simple records index
You do not need to sort every document perfectly before you begin. Start with one short index: a dated list of the records, notices, and notes you already have. Give each item a plain label that helps you recognize it later, such as the office or organization that sent it, the document type, and the date on it.
For paper documents, note the folder or envelope where you keep the original. For a portal download or digital file, note the folder, device, or account where you saved your copy. The goal is to make the location clear to you, not to create a second version of the record from memory.

- Reference, claim, case, or confirmation number when one is provided

Use categories that match the paperwork you receive
A few dependable categories are easier to maintain than a long list of folders. Keep appointment paperwork together so you can find visit summaries, scheduling messages, and questions you noted for an office. Keep each prior authorization request with its dates, notices, reference numbers, and the follow-up described by the plan or provider office.
Use a separate place for insurance calls. Include the call date, the representative or department, the reference number, a factual summary of what was said, and any next administrative step. This avoids relying on a scattered note or trying to reconstruct a conversation later.
For medications, keep only the information as documented by your providers, pharmacy, plan materials, or prescription label. This category is a record of documents and communications, not a place to decide what to take, change, or stop. Keep letters, portal messages, bills, forms, and other correspondence in their own category with the date and sender clearly labeled.

Add dates and next steps without filling in gaps
When a notice or conversation gives you a date, write it down exactly as provided. A single timeline can include the date a document was received, a call was made, an appointment was scheduled, or a follow-up was requested. If wording is unclear, keep it as a question for the organization that issued the notice instead of trying to turn it into an answer.
You can also add a short status note such as “waiting for a copy,” “asked plan for reference number,” or “follow-up date listed in notice.” These labels describe your own administrative progress; they do not interpret the record, coverage, or care. Review the index after new paperwork arrives so the system stays useful without becoming a major project.
